Family Member Eligibility Reverification

RE: Family Member Eligibility Reverification

To ensure that UC’s benefits resources are used according to plan eligibility, periodically UC will ask plan members to reverify the eligibility of some family members enrolled in Health and Welfare benefit plans.  Family members included in the reverification may consist of your spouse, domestic partner, stepchild, grandchild/step-grandchild, legal ward or overage disabled child.  Biological, adopted, and foster children are not subject to the reverification process. 

What Employees Should Expect

Employees who verified family members more than three years ago will receive a packet (includes a Spanish version) of materials from UnifyHR, UC's third party vendor,  with detailed instructions on how to complete the reverification process. 

Action Required

Employees must respond by the deadline specified on their letter or risk the disenrollment of their family members from UC benefits.
