Topic:  The Essence of Advanced Care Planning

Date:  Thursday, May 3rd, 12:00 – 1:00, in the HR Learning Center (3101 SAASB)

No registration required.  Open to all UCSB employees. You are welcome to bring your lunch. 

Advanced Care Planning (ACP) is the two-step process of
     1. Deciding what you do/don’t want in terms of medical care at end of life, and
     2. Documenting and specifying in an Advanced Care Directive (ACD) who will communicate those thoughts and desires to medical staff when you cannot.This presentation will discuss ACP in relationship to other life planning, decisions, who would benefit from engaging in the process, what happens when the ACD is in effect, and recommendations for the decision making process.  Materials will be provided to participants to assist in beginning this process of reflection on major medical care decisions for yourself and your families.

Presenter: Herm Trabbic
Herm Trabbis is a retired Hospital Staff Chaplain and worked at Howard County General Hospital in Maryland, primarily in the ICU and oncology departments.  He was also a member of the palliative Care Team there.  He earned a Master’s degree in Theological Studies from Washington Theological Union in D.C.  Additionally, Herm holds verifications as a trainer on Maryland’s Medical Orders for Life Sustaining Treatments, in Palliative Care Chaplainry from Cal State San Marcos, and as an Advanced Care Planner by the Alliance for Living and Dying Well, Santa Barbara, CA.  Based upon his experience as a Hospital Chaplain, he has great motivation and passion for educating others on the importance of Advanced Care Planning.
