SUBJECT: Behavioral Health Transition of Care Benefit

This is a reminder that United Behavioral Health (UBH) is now providing
mental health and substance abuse care for all UC medical plans (except
Core). All outpatient behavioral health care must be pre-authorized by
calling UBH Member Services before seeking services.

If your current provider is NOT contracted with UBH, you and covered family
members may be eligible for a special transition benefit. Please see notes
below for details about this benefit.

Call UBH by JANUARY 31, 2008 to apply for the transition of care benefit.

Health Net members will be transitioning from MHN to United Behavioral
Health (UBH) for mental health and substance abuse treatment. If your
Health Net/MHN outpatient behavioral health provider is NOT a UBH provider,
you may be able to continue with your current non-participating provider
through UBH at the in-network benefit level until 6/30/08.

You must have been receiving mental health or substance abuse outpatient
services prior to 1/1/08 and extending in to 2008.

Blue Cross Plus and CIGNA (formerly Definity Health) members will no longer
have an out-of-network option. In 2008, all behavioral health benefits
must be delivered by a UBH provider.

If your outpatient behavioral health provider is not a UBH provider, you
may be able to continue with your current non-participating provider
through UBH at the in-network benefit level until 6/30/08.

You must have been receiving mental health or substance abuse outpatient
services prior to 1/1/08 and extending in to 2008.

Blue Cross PPO member will continue to have access to UBH and non-UBH
providers.

If you are seeking care from a non-UBH provider, please call UBH to tell
them the name of your practitioner. There is a 20 visit limit per calendar
year for non-UBH practitioners.
