Benefits Information Packet for the Employee

Please provide the following documents to your employee at the time of the layoff notice and encourage them to contact the Benefits Representative listed on their letter for  questions concerning this information.

COBRA Rates
In order to continue the medical, dental and vision coverage, the employee will need to apply for COBRA. The COBRA application packet will be sent to the laid off employee from CONEXIS (the University's third party COBRA vendor) approximately 30 days from the date of the separation.  Premium information is shown on the attached form.
